Error description
On VIOS versions 2.2.3.0 and higher, when the VIOS is booted, there is a small chance that some LV-backed or file-backed virtual disk devices do not get configured. The problem may be consistent for a particular system, i.e. if the problem does occur on a particular system, it may occur all or nearly all the time on that system.
