Error description
The CN4093 has multiple Fibre Channel Forwarders (FCFs) internally. If one fails or is disabled, failover may not happen properly to the other FCF. A CVL notification followed by and FCF dead indication from the adapter would experience this problem.

The CN4093 has multiple Fibre Channel Forwarders (FCFs) internally. If one fails or is disabled, failover may not happen properly to the other FCF. A CVL notification followed by and FCF dead indication from the adapter would experience this problem.
Problem conclusion
Pay attention to FCF Dead notifications any time we are in the link/connection establishment phases of the adap init state machine. Previously we would only pay attention if we had an actively-selected FCF, but this does not catch the CVL followed by FCF Dead case (CVL will clear SELECTED, which was causing us to drop the FCF Dead notification).
