Error description
If an LPM is performed with NPIV Disks using single_path reserve policy, and hardware issues are encountered on the SAN fabric or Storage Subsystem, then this may result in failure to break the disk reservations. If the disk reservations are not broken after LPM completes, this will lead to disk I/O failures.

Problem conclusion
In some cases, the hardware issues encountered after the LPM operation may be recoverable with some additional retry of SCSI commands and LUN Reset operations. The Disk Device Driver code was updated to allow for more retries of certain SCSI commands and LUN Reset commands after a LPM, to allow for additional login attempts to the Storage Subsystem. Note, severe issues on SAN Fabric and Storage Subsystem may still result in reservation conflicts or I/O failures after a LPM.
