Error description

• The "Save and Approve" button is only available when editing
  content in the first workflow stage.
  
  The "Save and Approve" button was only being added to the
  toolbar if the content was in the first workflow stage.
  

Local fix

• The "Save and Approve" button is now available in all workflow
  stages except the last where the user has both edit and approve
  access.
