Error description
In certain cases, such as if you have a post-process property mod on a lifecycle request, and if that request fails, an event is added to the document's history. This is a problem because the request will be processed (and fails) repeatedly by the lifecycle service, hence creating repeating entries in the document's history.
Local fix
use pre-process property mods.
