Well, I gues the view is gone now...but what might have happened is that
something may have gone wrong during the mkelem operation without the user
noticing for some reason. The mkelem on an existing file is done in stages
and at one point the original file (say, 'foo.c') is renamed to
foo.c.mkelem, the element is created and checked out and then replaced with
the foo.c.mkelem file contents. For example, CC might get a problem with
checking out the element for some reason and thus you will now see a file
element with the expected name but no content while the original file and
contents can be found as foo.c.mkelem.

Aside from the occasional event like the described, I've never seen data get
lost (and in the above event, it has been fully recoverable).

Well, just a thought - maybe the view is still around for you to check if
this is possible...
